Information Update from  Ministers

CoIC Ministers are now  required by law to keep their contact information up to date.  We are providing this online form for convenience. Any information entered here is automatically emailed to the executive ministers.  Information can be mailed in as well.   Click here for a form to mail in.  The church is also required to keep on file a copy of a current form of Photo ID such as a Driver's License or Passport.  We ask that you fill out this form completely and in the comments section briefly tell us what you have been doing in your ministry this past year. (Please note, this is strictly a formality.  No one will be accessed on their comments.)  If any section does not apply to you, please type (NA) in the appropriate field.  Submit button for the form is all the way at the bottom of webpage.
